At Haven, we provide comprehensive annual exams, fertility support, specialty testing, and gynecologic care in a setting where you'll have the time and space to ask questions, discuss concerns, and better understand your health.
About Our Nurse Practitioner
Katherine Jacobs, RN, MSN, FNP-BC
Katherine Jacobs is a Family Nurse Practitioner with a passion for helping women better understand their health and feel confident in their healthcare decisions.
She provides comprehensive annual exams, fertility support, specialty testing, and care for basic gynecologic concerns. Her approach combines evidence-based medicine with the time, attention, and personalized care that are often missing from traditional healthcare settings.
Katherine believes that meaningful healthcare starts with listening. By taking the time to understand each woman's history, concerns, and goals, she helps her patients make informed decisions and develop practical plans that support their health and well-being.
At Haven Birth and Wellness, Katherine provides care in a calm, relationship-centered environment where women have the space to ask questions, explore options, and feel truly heard.
Women's Wellness Services
Comprehensive Annual Exam
$350
This comprehensive visit goes beyond a routine annual exam. Along with preventive and gynecologic care, you'll have dedicated time to discuss your health, concerns, symptoms, and goals.
Includes:
- One 60-minute annual exam
- One 30-minute follow-up visit to be used within 12 months
Your annual exam may include:
- Review of health history, medications, and supplements
- Discussion of menstrual cycles, hormones, fertility, and reproductive health
- Basic physical exam and vital signs
- Pap smear and/or pelvic exam when indicated
- Breast health screening recommendations
- Preventive screening recommendations based on age and risk factors
- Lab recommendations as indicated
- Personalized recommendations around nutrition, exercise, sleep, stress, and overall wellness
- Time to discuss symptoms, concerns, and health goals
Your included follow-up visit can be used for lab review, questions that arise after your visit, or a problem-focused appointment.
Annual exams establish care with Katherine, allowing you to schedule future problem visits and receive ongoing wellness care. To remain established, annual exams must be completed every 12 months.
Fertility Support
$550
Personalized fertility support for women preparing for pregnancy, trying to conceive, or navigating fertility challenges.
Includes:
- Complimentary 15-minute consultation
- One 60-minute fertility intake visit
- Two 30-minute follow-up visits to be used within 12 months
Fertility Support begins with a complimentary consultation. After completing an intake form, you'll meet with Katherine to determine whether this service is appropriate for your situation and goals before making any financial commitment.
This service may be appropriate for women who:
- Are preparing for pregnancy
- Are actively trying to conceive
- Have PCOS or endometriosis
- Are seeking fertility support over age 40
Because fertility care rarely fits into a single appointment, this service includes ongoing support through follow-up visits as questions arise, testing is reviewed, and recommendations are adjusted.
Your fertility intake visit establishes you as an active wellness client, allowing you to schedule future problem visits and receive ongoing care related to your reproductive health.
If you become pregnant before using all of your included follow-up visits, any unused visits may be applied as a $150 credit toward other Haven services, including but not limited to a homebirth global fee, functional testing, or IV hydration.
Specialty Testing & Review
$250 + testing fees
For women interested in specialty testing and professional interpretation of their results.
Includes:
- One 15-minute testing consultation
- Test selection and ordering
- One 45-minute results review appointment

- Genova Diagnostics IgG Food Sensitivity Testing - Starting at $129

Appointments may be completed in person or virtually for Tennessee residents.
Specialty testing appointments focus specifically on test selection, interpretation of results, and recommendations for possible next steps. They do not replace comprehensive wellness care and do not establish care with Katherine.
Additional tests reviewed during the same appointment require an additional 15 minutes of review time per test ($50 each).
Looking for care for a specific concern?
Established wellness clients can schedule problem visits for women's health concerns such as urinary symptoms, vaginal concerns, breast concerns, menstrual concerns, fertility concerns, and more.
If you're new to Haven Wellness, your first step is to schedule either a Comprehensive Annual Exam or Fertility Support. These visits establish care with Katherine and provide time to discuss your current concerns and determine appropriate next steps.
